Transfer of Psychological Files: Request Form

Why this form may be helpful for continuity of care


When people change psychologists, having access to past psychology records can be beneficial for the new clinician.


Having copies of previous notes and reports can help by:


- Building a clear picture of your history without you having to repeat everything from scratch.

- Understanding what strategies, approaches, or ideas have already been tried.

- Avoiding unnecessary duplication and helping us use session time more effectively.


Any information received is used as background only. We will always check important details with you and discuss what feels accurate, relevant, or no longer fits.


Past records don't replace your voice or your current experience.


About your privacy and choice


Under Australian privacy law, you generally have the right to access your own health records. This form simply helps you make that request in a clear and straightforward way if you choose to do so.


Using the form is optional. Some people find it helpful for the transfer of files; others prefer to start fresh. There is no expectation that you request past records unless it feels useful for you.


If you’re unsure whether requesting records is right for your situation, we can talk it through together.


This form is intended to support continuity of care, not to create conflict with previous providers.
